Medical Action Plan Update

Student Medical Action Plans are required to be updated every year. This includes asthma, anaphylaxis, allergies, diabetes and more.

The medical action plan should be renewed and signed by a medical/health practitioner and then supplied to the College to be put on file.

Copies of the forms are available in hard copy at Student Services and are attached below:

• ASCIA Allergy Action Plan
• Asthma Action Plan
• Asthma Care Plan
• Diabetes Action Plan 2020
• Epilepsy Management Plan

All completed forms must be returned to Student Services or sent to bendigo.southeast.sc@education.vic.gov.au.
