Student Health Care Needs – Bendigo South East College

 At Bendigo South East College, the health, safety and wellbeing of every student is a priority. To support students effectively and meet our duty-of-care obligations under Victorian Department of Education (DET) guidelines, families must provide current medical information and documentation where a student has identified health care needs.

 If your child has a medical condition — including asthma, anaphylaxis, allergies, diabetes, epilepsy, or other ongoing health needs — the following documentation is required as a matter of urgency:

 Required documentation

  • A current Medical Management / Action Plan completed and signed by a medical practitioner
  • The College Health Care Needs Form / Student Medical Information Form
  • A Medication Authority Form (if medication is required at school)
  • Medication supplied in original packaging, clearly labelled and within expiry date

 Students diagnosed with asthma or anaphylaxis must have a current action plan on file to safely participate in school activities, including camps, excursions and sporting events.

Families can access the relevant DET templates below:

 Completed forms and medication should be provided to Student Services or the General Office. Families are also asked to notify the College promptly of any changes to their child’s health needs and supply updated documentation as required.
